1. Academic Transcripts

Recommended: That, effective September 1, 1999, the policy on Academic Transcripts be revised to permit students to request a partial transcript restricted to marks and grades leading to a specific degree program, as set out below.

Recommended: That, effective September 1, 1999, the policy on Academic Transcripts be revised to add post-1993 information on scholarships, awards, fellowships, prizes and medals.

Academic Transcripts (S.96-85, Sen.Ag. June 19/98)

A transcript is a copy of a student's permanent academic record at this University, duly certified by the Registrar and bearing the embossed seal of the University. A transcript is privileged information and is available only upon the written request and payment of the fee in effect at the time by the student.

A transcript is required as one of the supporting documents for application to another university, graduate school, fellowship and scholarship applications, and is commonly required by prospective employers.

The transcript is a record of a student's academic progress. It contains the following information:

1. A listing of all courses attempted and the grades achieved, including courses from which a student has withdrawn without academic penalty.

2. A statement of the degree attained, including the area of concentration or Honors discipline and date of graduation.

3. Comments relating to a student's academic progress. These may include statements about a student's standing in a program (e.g., on Dean's Honor List), or that the student was required to withdraw from the University or was placed on academic probation (e.g., for failing to meet progression requirements).

4. A listing of all scholarships, awards, prizes, fellowships and medals awarded by the University to the student during the student's academic career at the University.

Note that a transcript reflects the current status of a student's record at the time it is issued. Students should ensure that any changes to the transcript (e.g. from an INC to a final grade) are recorded before ordering a transcript.

A student may submit a written request for a partial transcript restricted to marks and grades leading to a specific undergraduate degree in Business, Dentistry, Education, Engineering Science, Law, Medicine, Nursing, Physical Therapy or leading to any graduate degree. Such transcripts shall be identified as partial.

Transcript order forms are available from Information Services, Registrar's Office, Stevenson-Lawson 190*. Official transcripts are mailed by the Registrar's Office to institutions designated by the student. Each transcript costs $x. Same-day transcripts are available at a cost of $x each.

* All transcript transactions in Room 190 require valid identification.

1. Report on New Undergraduate Scholarships/Awards/Bursaries

SCAPA has approved on behalf of the Senate the following Terms of Reference for new awards and bursaries for recommendation to the Board of Governors through the Vice-Chancellor:


Gordon Jeffery Endowment Strings and Chamber Music Continuing Awards (Faculty of Music)
Awarded annually to students enrolled in any year of a Faculty of Music program. Students must demonstrate outstanding performing ability with preference given to orchestral string players (violin, viola, cello, bass). Selection and award amount will be made by the Faculty of Music Scholarship Committee. These awards were made possible by a bequest from Gordon Jeffery (BA '40).
Value: Up to $4,000 each

Jeannetta Marion Burroughs String Award (Faculty of Music)
Awarded to an undergraduate student of violin, viola, violoncello, or double bass, enrolled in any year of a Faculty of Music program. The student will be chosen on the basis of performance excellence, potential and demonstrated financial need. Final selection will be made by the Faculty of Music Scholarship Committee. This award has been created through generous donations from Patrick Burroughs, Music Mart Pro, Remenyi House of Music, Walters Music (Bramalea) Inc., and Robert Skelton on behalf of the Canadian String Teachers' Association, in memory of Jeannetta Marion Burroughs.

Doris Jackson Memorial OSOTF Award in Music (Faculty of Music)
Awarded annually to the student in the Faculty of Music who has made a significant contribution to the UWO Jazz Ensemble and demonstrates financial need. This award was established by the London Jazz Society as a memorial to its founding president.

McCormick Canada Continuing HBA Award (Richard Ivey School of Business)
Awarded annually to a full-time student entering the first year of the HBA program. The student must have a minimum 'B' average, demonstrated community and/or extra-curricular involvement and financial need. Students may retain this award in year two provided they continue to meet the criteria. If a student fails to retain the award, other students entering year two will have an opportunity to apply for this award. This award is made possible as a result of the generosity of McCormick Canada Inc.

Students must apply for this award. Application forms are available from the Admissions Office of the Richard Ivey School of Business and must be completed and returned along with a current resume to the Admissions Office by August 1. Final selection will be made by the HBA Scholarship Committee of the Richard Ivey School of Business.

ING Bank of Canada Continuing HBA Scholarship (Richard Ivey School of Business)
Awarded to a student entering the first year of the HBA program with a minimum 90% average who has demonstrated community leadership. Preference will be given to a student who has indicated interest in pursuing a career in integrated financial services (this includes insurance, banking, asset management, securities, and bancassurance). The award will be renewed for year two for students maintaining the defined criteria in HBA year one. If a student does not qualify to retain the award, other year two students will be given the opportunity to apply. This award has been created to recognize and reward academic achievement and leadership in Ivey HBA students and is the result of a generous donation from ING Bank of Canada.

Application forms are available from the Ivey Business School Program Services and Admissions Office. Completed application forms must be accompanied by a short essay (maximum 500 words) describing the student's community leadership experience. Application deadline is August 1. Final selection will be made by the HBA Scholarship Committee of the Richard Ivey School of Business.

Sam Katz OSOTF Bursary (Any Faculty)
Awarded to an undergraduate student with demonstrated financial need. Preference will be given to students of Danish descent.

McMillan Binch Scholarship (Faculty of Law)
Awarded annually to the student who has achieved highest overall academic standing in first year law. Final selection will be made by the Faculty of Law Scholarships Committee. This scholarship was made possible by a generous donation from McMillan Binch.

Faculty of Music Alumni Bursaries (2) (Faculty of Music)

If the student does not meet these requirements, year two students will have an opportunity to apply for this award. The HBA Scholarship Committee of the Richard Ivey School of Business will review and select each award recipient. Application forms for this award are available from the Admissions Office of the Richard Ivey School of Business and must be completed and returned to the Admissions Office by September 15. The recipient will be nominated by faculty members in the School of Nursing, and final selection will be made by the Nursing Scholarships and Awards Committee. This award was made possible by the generous donations of family and friends, in memory of Alfreda Simons Kartha, a distinguished faculty member in the School of Nursing. Candidates must have a minimum 85% average and demonstrate financial need. Applications can be obtained from the Office of the Registrar and must be completed and returned by October 31. David Jackson (BSc '46, DSc '89) in memory of Professor Gar Woonton who served the UWO Physics Department from 1931 to 1948 before going to McGill and later Laval. 2. Withdrawal of the Bachelor of Science (Occupational Therapy)

SCAPA has approved the discontinuation of the program leading to a Bachelor of Science (Occupational Therapy). The discontinuation of the BSc(OT) is tied to the introduction of the Master of Clinical Science in Occupational Therapy which will replace it. The proposal is detailed in the Report of the Senate Committee on University Planning (Exhibit III, page 1).
